Case Study
Our experienced incident investigation team enabled assisted NHS England with an investigation into the care and treatment of 14 patients who underwent child heart surgery. We recruited an expert advisory panel to help us formulate and validate our findings, and produced an overarching report summarising previous investigations, setting out lessons to be learned and recommending further actions which would improve a range of service areas both locally and nationally.

We examined systems and processes to identify any areas of vulnerability as well as notable good practice at an acute foundation trust, post-merger, to understand if there was an underlying systemic cause of 4 ‘never events’ and whether action was needed to be taken to safeguard patients. Our investigation into the incidents identified a fundamental issue resulting in a lack of cohesion across the 3 hospital sites in terms of processes and policies, and a weak and fragmented clinical leadership structure.
