Featured case study: Overseeing national safeguarding scrutiny
Verita’s capacity to handle high-profile, sensitive safeguarding investigations is built on a proven track record of managing complex, public-facing inquiries.
A prime example of this is our work supporting Kate Lampard in overseeing investigations into matters relating to Jimmy Savile across NHS organisations. Commissioned by the Secretary of State for Health, Verita devised an assurance framework to audit the robustness and integrity of 32 high-profile NHS reports, ensuring that investigations into deep-rooted safeguarding failures could stand up to intense scrutiny from victims, the media, regulators, and the general public.
This landmark project culminated in a comprehensive lessons-learned report for the government, directly shaping modern healthcare leadership, security arrangements, and governance standards around safeguarding.
Other notable safeguarding work includes the independent investigation into the Green Party’s actions following the allegations and charges brought against David Challenor, which revealed a low level of awareness of safeguarding in the Party, and provided actionable recommendations on improving safeguarding policies and procedures.
What Our Clients Say About Us
“Verita is the undisputed market leader for investigations and reviews.”
“We would highly recommend Verita. We appreciated their professionalism in dealing with a sensitive issue expertly and discreetly, and doing so in a timely fashion.”
Why choose Verita: Expertise & peer review
When a safeguarding investigation is complex, working with an organisation with extensive experience is paramount.
Verita operates with a multi-disciplinary team of senior associates and industry specialists who peer review our investigation work internally.
This extra layer of scrutiny ensures that our evidence gathering, anayltical findings and governance recommendations are thoroughly tested, completely objective, and fully defensible under public or regulatory scrutiny.

What We Say
“We know that many other organisations will be outwardly confident that they have appropriate safeguarding policies and procedures in place. But this confidence may be misplaced if the leaders of those organisations cannot be sure that everyone who works for them knows who is responsible for safeguarding and is empowered to speak and act when they identify concerns.”

Safeguarding Investigation FAQs
Handling an independent safeguarding investigation effectively requires establishing clear, objective Terms of Reference from the outset and engaging specialised external investigators with deep sector expertise. The process involves conducting thorough, trauma-informed evidence gathering and interviews that look beyond written policies to uncover operational realities and subtle “weak signals” of inappropriate behavior. To ensure the inquiry withstands legal, regulatory, and public scrutiny, all findings should undergo rigorous peer-reviewed oversight, culminating in actionable recommendations that address governance vulnerabilities and rebuild organisational trust.
Safeguarding is everyone’s responsibility, requiring every employee, volunteer, and manager to actively report concerns or inappropriate behaviours. However, boards, executives, and HR leaders are responsible for setting robust strategy, establishing safe operating systems, and enforcing strict codes of conduct. When an incident occurs, leadership is ultimately accountable for ensuring the matter is independently and thoroughly investigated, that “weak signals” or systemic failures are addressed, and that the organisation fulfills its moral and statutory duty to protect vulnerable individuals from harm.
An organisation should instruct an external, independent safeguarding investigator when allegations involve senior leadership, trustees, or HR personnel to eliminate any perceived or actual conflict of interest. External expertise is also essential during complex or historical disclosures, following formal notices or inquiries from regulators such as the CQC, Charity Commission, or Ofsted, or when internal teams lack specialised safeguarding capacity. Bringing in an independent partner provides objective, peer-reviewed scrutiny that delivers unassailable findings, reassures regulators, and restores public and board confidence.
