Systematic incident investigation

This practical, interactive course shows participants how systematic investigation can be used to discover why an incident occurred and identify solutions to prevent similar incidents happening again. The programme covers the six key stages in the RCA process for serious incident investigation:

  • reporting the incident
  • finding and securing the evidence including mapping information
  • analysing data
  • developing recommendations
  • implementing solutions
  • evaluation.

Participants are taught in facilitated working groups using practical examples. One and two-day courses are available.
