A one-day, interactive training course for designated safeguarding leads and IMR authors

Expectations for serious case reviews are increasing all the time. IMR authors need to meet the same exacting requirements or risk undermining the quality of the overall serious case review - or even worse the IMR and related serious case review having to be done.

Verita has developed a practical, interactive one-day course to help all those responsible for undertaking IMRs to get it right first time.

The course focuses on the review process, helping authors to conduct systematic, evidence-based investigations that are proportionate for the case under review. It covers every stage of the investigation from shaping the terms of reference to ensuring recommendations are clear, measurable and lead to meaningful change.

Participants work in small groups using case studies and examples to explore:

  • the theory behind why things go wrong and human error
  • systematic investigative processes and techniques and how to use them
  • how to develop a chronology of events
  • how to interview staff and provide an enduring record
  • what constitutes evidence and the different types of evidence
  • analysing evidence and drilling down to findings
  • writing a good report with clear, measurable recommendations.
