Verita has identified serious failings at a residential home for adults with autism.

The home is run by a charity that provides a range of services and residential facilities for people with disabilities.

Trustees and the chief executive of the charity approached Verita to carry out an independent investigation into concerns they had about clinical standards at the home.

Verita partner Barry Morris and associate Jane Held carried out the investigation. Barry Morris describes what they found: "The fundamental problem was a lack of clinical and management leadership and accountability. This coupled with poor governance systems, lack of management scrutiny and staffing problems led to a number of failings in care."

Verita put forward a range of practical recommendations to drive up standards of care. These included establishing clear management responsibility and accountability, developing and monitoring a range of performance indicators and reviewing staffing levels.

The investigation was completed within three months.
